Dr. Krishna C. Chunduri. M.D.
Anesthesiologist & Pain Management Physician
Krishna C. Chunduri, M.D. is board certified in Pain Medicine and Anesthesiology, and is currently the Director of Pain Medicine at Saint Anthony Hospital and formerly the Chief of Anesthesia. He graduated from the University of Illinois, Urbana-Champaign with a degree in Biology, and then completed medical school receiving his medical degree from St. Georges University. At St. Louis University he completed a transitional internship followed by an anesthesiology residency where he took keen interest and trained extensively in pain management. Dr. Chunduri’s specialty interests include interventional pain medicine, treatment of chronic pain, and effective treatment of the injured worker so as to ensure rapid return to the work place.
Dr. Phillip A. Gattas D.C.
Dr. Phillip A. Gattas is a Board Certified graduate of Logan College of Chiropractic. He has been in practice in the Chicagoland area since 1990. For the greater part of those years, Dr. Gattas’ specialty has been the effective treatment of the injured worker in order to ensure a rapid return to the workplace. Besides helping the injured, he loves to read, and has had an interest and participated in such sports as bicycling, skiing, golf, archery and fencing. He has also been known to ride his motorbike when he can. His philosophy has always been to provide excellent patient care, while promoting optimal healing and wellness. Dr. Gattas is fluent in English and Spanish.
Dr. Oleh S. Paly M.D.
Physical Medicine and Rehabilitation
Dr. Paly was born in Chicago and completed his undergraduate studies at Loyola University of Chicago. His medical studies, as well his specialty training, were completed at The Chicago Medical School. Board Certified in Physical Medicine and Rehabilitation, Dr. Paly is a clinical assistant professor at The Chicago Medical School. His professional affiliations include the American Academy of Physical Medicine and Rehabilitation, the American Medical Association, the American Association of Electrodiagnostic Medicine, and others. Dr Paly has been performing electrodiagnostic testing in the Chicago area for over 15 years. He is fluent in Polish, Spanish, and English.
Chris Fortier P.T.
Chris is a local resident who grew up on the south side of Chicago. After receiving his bachelors degree from Northern Illinois University, he attended Midwestern University in Downers Grove, Illinois where he obtained his Masters Degree in Physical Therapy. Chris is proficient in Spanish and his areas of interest include sports medicine, general orthopedics and geriatrics. Having worked at Lutheran General Hospital’s outpatient clinic, Chris has experience with a wide range of musculoskeletal problems and a variety of patient populations. Recent continuing education that influences his practice include Gary Grey Functional Rehabilitation and Mulligan Mobilizations with movement.
Theresa Buk P.T.A
Licensed and Board Certified in Physical Therapy
Theresa Buk, (Teri) grew up in Cicero, the seventh of eight children. She received her Physical Therapy Assistant’s degree at Morton College, and her B.A. from the University of Illinois in Chicago. She has experience working in a variety of Physical Therapy settings, including acute care hospitals, acute and sub-acute rehabilitation facilities, aquatic physical therapy facilities, outpatient clinics with the emphasis on orthopedic rehabilitation, and home health services, actually treating patients in their home environments, to provide physical therapy services to people who are unable to get out to receive outpatient clinical services. Teri enjoys working with her patients and their Doctors to provide comprehensive P.T. treatment and ensure the best results possible for her patients. In her free time, Teri likes to travel swim, snorkel, ski, hike, cycle and camp. She is an avid gardener. She lives an active lifestyle and loves the outdoors.
